Direct moxa is often done in tandem with acupuncture, but not always. If needles are used, the cones are placed either beside or near the needles. Some will target a spot by placing needles around a spot, then place the cone in the middle.

The doctor will make tiny moxa cones and apply them on specific points before lighting them, usually with a smoldering incense stick. The cones will burn quickly and singe the skin, leaving a black ash behind. If your doctor is really good, the cone will have burned out before you even feel the burn.
